Working on the classification and structure diagnosis of cytomegalovirus infection in children of early age

AbstractThe authors observed 194 the child's first year of life with mono - and mixed-cytomegalovirus infection (CMVI): 178 children with mono-cytomegalovirus infection (CMVI), 16 children with mixed infections (4 - CMVI combined with viral hepatitis, 12 - CMVI combined with infection caused by herpes simplex virus 1, 2 type). Etiological diagnosis was made on the basis of the results of the study of material from patients by PCR, detection of CMV-cells in saliva and urine, diagnostic titers of specific antibodies of class IgM and IgG by ElISA. In addition, the study 14 reports the pathologic-anatomic study of 12 deceased children at the age from 6 days to 2 years and 2 fetuses (1 antenatal death of the fetus in the gestational age of 32 weeks and 1 miscarriage induced late in the period 23-24 weeks), in all cases, the diagnosis of cytomegalovirus infection, confirmed by histopathology (characteristic of the detected cells in the form of «owl eyes» in parenchymatous organs and of the salivary glands), including 8 cases were found to have a generalised infection, 6 - diagnosed multiple malformations in combination with generalized cytomegalovirus infection.

Based on these data, the analysis of the dynamics of the disease in patients with cmvi, including when observing some of them at the outpatient stage and results of pathoanatomical research, the analysis of the current cmvi classifications presented in the literature, and taking into account the principles of classification of infectious diseases according to A. Koltypin, designed additions to the working classification of cytomegalovirus infection in children of early age. When forming patterns of diagnosis of cmvi in children of early age should take into account the period of onset, course, clinical form, presence of markers, the stage, severity, options for associations with other diseases and disease outcomes.
