Acute diarrheal infections: principles of rational therapy

Abstract

Acute diarrheal infections (ADI) - a group of infectious diseases, with more than 30 nosological forms, pathogens which can be viruses, bacteria and protozoa. Currently, the etiological structure of ADI is dominated by viruses. Among bacterial ADI, food intoxication, salmonellosis and shigellosis are most often recorded. A practitioner for adequate etiotropic therapy is important to be able to differentiate shigellosis, campylobac-teriosis, diagnose cholera in a timely manner, as the treatment regimen of shigellosis and campylobacteriosis involves the appointment of etiotropic therapy. Treatment of patients with other diseases of this group in most cases is limited to pathogenetic and symptomatic means. In case of verification of the diagnosis of cholera, emergency anti-epidemic measures are required.

The principles of treatment of patients with ADI provide for the simultaneous solution of several clinical problems, which are aimed at preventing the further development of the infectious process and possible complications. The choice of tactics of treatment of ADI determines a number of factors. First of all-the form and severity of the disease, the patient's age, premorbid background.

The article presents the basic principles of etiotropic and pathogenetic therapy of ADI, draws attention to the priority of pathogenetic and symptomatic links, including the use of racecadotril in combination with rehydration therapy.
